As a key member of our care team, you will be responsible for implementing the patient's prescribed plan of care, conducting patient assessments, and coordinating care with the patient's healthcare team.
Responsibilities- Implement Physician's Prescribed Plan of Care: Develop and implement a comprehensive care plan that meets the patient's unique needs and goals.
- Patient Assessments and Coordination of Care: Conduct regular patient assessments to identify changes in the patient's condition and coordinate care with the patient's healthcare team.
- Administration of Medication and Treatments: Administer prescribed medication and treatments as ordered by the patient's physician.
- Compliant and Timely Documentation: Maintain accurate and timely documentation of patient care in our electronic system of record.
- Health Promotion and Education: Educate the patient and their family members on healthy habits and disease management.
- Verbal Orders and Communication: Accept and write verbal orders from physicians as required and obtain co-signature of Case Manager on all verbal orders.
- Follow-up and Communication: Follow up with the patient, their family members, and the healthcare team on patient problems, concerns, and changes in patient condition.
- Active RN License: Must have and maintain an active, unencumbered RN license in the state in which you will practice.
- CPR Certification: Current CPR certification from the American Red Cross or American Heart Association.
- TB Skin Test: Current TB skin test within the last 12 months.
- Reliable Transportation: Must have reliable transportation to and from patient locations.
