RN Case Manager

Job Summary:
The SCF RN Case Manager is responsible for delivering high-quality, patient-centered care to customer-owners, promoting cost-effective health care outcomes while adhering to established case management standards and Advance Access principles.
Key Responsibilities:
- Develop and implement individualized care plans to meet the unique needs of customer-owners.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal health outcomes.
- Provide education and support to customer-owners and their families to promote health literacy and self-management.
- Participate in quality improvement initiatives to enhance the overall quality of care.
Requirements:
- Graduate of an accredited school of nursing.
- License in the State of Alaska as a Registered Nurse.
- Basic Life Support (BLS) certification is required. ACLS, NRP, ATLS, ALSO, and PALS as required for specialty.
- One (1) year nursing experience or successful completion of the RN Case Manager Training.
Preferred Qualifications:
- Under P.L. 93-638, as amended, the company pursues a policy of Native preference in hiring, contracting, and training.
