RN Case Manager

We are seeking a skilled RN Case Manager to join our team at Travel Nurse Across America. As a RN Case Manager, you will play a critical role in advocating for patients, evaluating their needs, and developing comprehensive care plans.
Key Responsibilities:- Evaluate and assess new patients to identify their needs and develop individualized care plans.
- Create and update comprehensive short and long-term patient care plans to ensure the best possible outcomes.
- Educate patients and their families about resources and healthcare options to empower them to make informed decisions.
- Collaborate with other healthcare professionals, including doctors and social workers, to ensure seamless care coordination.
- Properly document and submit medical records to ensure accurate and timely communication.
- Communicate with insurance companies to resolve coverage issues and advocate for patients' needs.
- Analyze results, monitor variances, identify trends, and recommend actions to management to improve financial outcomes.
- BLS certification
- 2 years of experience in case management
- Additional certifications may be required before beginning an assignment
