Nurse Practitioner

Catholic Health Services is seeking a compassionate and skilled Registered Nurse to join our team in providing high-quality care to our patients and their families. As a Registered Nurse - Continuous Care, you will play a vital role in delivering patient-centered care, promoting health and well-being, and supporting the emotional and spiritual needs of our patients.
Key Responsibilities:- Assess and coordinate patient care, identifying and addressing physical, psychological, social, and spiritual needs.
- Develop and implement individualized care plans, collaborating with interdisciplinary teams and healthcare professionals.
- Provide education and support to patients and families, promoting self-care and empowerment.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) degree.
- Active Registered Nurse license in the state of Florida.
- 1-2 years of nursing experience, with hospice or palliative care experience preferred.
- Certification in Hospice & Palliative Medicine (CHPN) preferred.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
