Nurse Practitioner

Catholic Health Services is seeking a compassionate and skilled Registered Nurse to join our team in providing high-quality care to patients and families in Miami and Broward.
Job Summary:The Registered Nurse is responsible for identifying and coordinating patient/family needs and providing preventive and supportive care to the terminally ill patient/family unit. This role requires a strong understanding of hospice care principles and the ability to work collaboratively with the interdisciplinary team to provide comprehensive care.
Key Responsibilities:- Assess physical, psychological, social, and spiritual needs of patients and families within 24 hours of admission.
- Complete accurate and timely documentation of patient/family services and maintain accuracy of electronic medical records.
- Coordinate care with the interdisciplinary team and ensure availability of all resources.
- Collaborate with nursing home staff to initiate written plans of care and maintain professional management of hospice care.
- Participate in quality assurance and performance improvement projects as directed by the Clinical Team.
- Serve as a case manager and advocate for patients and families, collaborating with attending physicians, insurance case managers, and other healthcare professionals.
- Administer medications and treatments prescribed by attending physicians and/or hospice physicians.
- Attend and participate in interdisciplinary team meetings as required.
- Participate in hospice activities, such as memorial services and community education, as requested by the Team Manager.
- Provide orientation to new employees and ensure effective communication with the interdisciplinary team.
- Participate in on-call rotations and demonstrate adherence to Catholic Health Services policies and procedures.
- Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) degree.
- Clear and active Registered Nurse license to practice in the state of Florida.
- 1-2 years of nursing experience, with hospice nursing experience preferred.
- Certification in Hospice & Palliative Medicine (CHPN) preferred, with eligibility to obtain within two years of employment.
- Knowledge of computer office/clinical software and ability to read, write, and understand the English language.
