Job Summary

Key Responsibilities
  • Provide comprehensive primary patient care, including prevention, health maintenance, diagnosis, treatment of acute and chronic conditions, and follow-up services to patients under your care.
  • Document all patient-provider interactions following healthcare standards.
  • Participate as an active member of an integrated patient-centered team.
  • Support the organization as needed.
Qualifications
  • Graduate of an accredited Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners.
  • Must have a permanent and unrestricted license to practice as a nurse practitioner in Arizona.
  • Must have current board certification in family medicine and maintain current medication dispensing license (DEA).
  • At least 3 years of experience in a family practice or comparable setting is required.
  • Experience in a community health center setting is preferred.
  • Current ACLS certification.
Requirements
  • Must possess a valid and unrestricted Arizona driver's license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years.
  • No more than two at-fault accidents in the last three years.
  • Must maintain a valid Arizona driver's license during employment.
  • Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with Native Health.
  • Must pass substance abuse testing upon employment and submit to random testing during employment.
  • Must provide an updated immunization (IZ) record at the time of employment.
  • Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual report.
  • Must obtain a First Aid/CPR Card within (90) calendar days of employment and maintain a valid CPR Card during employment.
Knowledge, Skills, and Abilities
  • Strong knowledge of current evidence-based family medicine guidelines.
  • Skills in providing diagnostic, preventive, and/or therapeutic services to patients.
  • Ability to work effectively with staff members who have a variety of backgrounds and training.
  • Ability to plan own work and carry out assignments effectively.
  • Ability to communicate with others effectively both orally and in writing in working out solutions to problems or questions relating to work.
  • Proficient with computer-based applications such as the EHR and Microsoft Office.
  • Ability to establish rapport and gain the confidence of others.
  • Ability to function as a healthcare delivery team member.
  • Ability to assess quality patient care.
  • Ability to maintain appropriate documentation of patient medical records.
  • Must be able to work with confidential material without betraying the trust of patients, families, or the agency.
  • Knowledge of the cultural and social backgrounds of the Native American and under or uninsured populations.
  • Adherence to principles of confidentiality, scientific/academic integrity, informed consent, and ethical practice.
  • Skill in providing excellent customer service and support; organizing and prioritizing workload.
