Urban Health Plan is seeking a skilled Intake Associate to provide exceptional patient experience and support the delivery of high-quality healthcare services. As a key member of our team, you will be responsible for conducting intake for new patients, completing patient assessments, and ensuring patient confidentiality.
Key Responsibilities- Conduct thorough intake assessments for new patients, including screening questions and required documentation.
- Provide exceptional patient experience by ensuring timely and accurate completion of intake processes.
- Collaborate with healthcare teams to ensure seamless patient care and support.
- Maintain accurate and confidential patient records, adhering to HIPAA guidelines.
- Participate in quality improvement initiatives to enhance patient care and services.
- High school diploma or equivalent required.
- One year of experience in an office setting with clerical duties.
- Bilingual Spanish/English required.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
