Job Summary

The RN Case Manager plays a vital role in ensuring the best possible outcomes for our patients. As a member of our rehabilitation team, you will work closely with patients, families, and healthcare providers to develop and implement individualized care plans.

Responsibilities

  • Provide program orientation to patients and their families, including an overview of the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient and family education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of patient treatment plans that support their strengths, abilities, needs, and preferences.
  • Document findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of patients and families.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
