Responsibilities- Manage rehabilitation care to ensure optimum outcomes for patients
- Coordinate and direct care specific to the needs of each patient
- Collaborate with the rehabilitation program director and facility case management director regarding departmental functions
- Coordinate efforts within the rehabilitation team
- Associate Degree in Nursing or Nursing Diploma, required
- Bachelor's Degree in Nursing, preferred
- 2+ years experience in case management or 3+ years experience in clinical nursing, required
