We are seeking a skilled Home Health Physical Therapist to join our team. This role requires a minimum of two years of experience in physical therapy and a graduate degree from an accredited physical therapy curriculum. The ideal candidate will have strong communication skills, be able to work independently, and have a commitment to service orientation.

As a Home Health Physical Therapist at Kaiser Permanente, you will have the opportunity to work with patients in their homes, providing independent physical therapy services and teaching preventative and rehabilitative procedures. You will also develop and coordinate plans of care, provide case management, and participate in educational and clinical research activities.

To be successful in this role, you will need to possess knowledge of Medicare conditions of participation and OASIS, as well as clinically skilled to detect early warning signs of complications. You will also need to be able to differentiate emergent versus non-emergent patient care status and look at patients holistically, not just body part or functional ability.

This is a full-time position, requiring 40 hours of work per week, with regular reassessments and evaluations to determine effectiveness of treatment and progress towards goals. We offer a collaborative and dynamic work environment, with opportunities for advancement and growth within the company.
