Job Summary: We are seeking a skilled Home Health Physical Therapist to join our team in Newark, New Jersey.

About the Role: As a Home Health Physical Therapist with LHC Group, you will be responsible for providing high-quality physical therapy services to patients in their homes. You will assess and evaluate patient care needs, develop treatment plans, and perform interventions aimed at improving patient well-being.

Responsibilities:

  • Evaluate patient functional status, body systems, and physical therapy needs.
  • Consult with physicians to develop therapy plans of care.
  • Provide services within scope of practice, as defined by state laws, in accordance with plan of care, and in coordination with other healthcare team members.
  • Observe and report patient responses to treatment and changes in condition to nurse supervisors and/or physicians.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
  • Instruct and inform participating healthcare team members, patients, and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
  • Visit patients within entire service area of respective agency and nearby LHC agencies as needed.
  • Complete progress notes per visit and submit per LHC policy.
  • Complete all patient evaluations and develop PT plans of care within state-specific guidelines.
  • Participate in staff conferences and committees as requested by agency.
  • May supervise home health aides per LHC policy if allowed per state regulation.
  • Provides supervision of PTA as per LHC policy and state regulation.

Requirements: To qualify for this role, you must have a current Physical Therapy licensure in New Jersey, Current CPR certification, and access to a dependable vehicle. Schedule flexibility is required, including potential availability on observed holidays, to ensure meeting demands of patients.

Estimated Salary: $85,000 - $110,000 per year, depending on experience.
