Key Responsibilities:
  • Develop and implement evidence-based care practices into protocols/policies, consultation strategies, staff development plans, continuous quality improvement initiatives, and research.
  • Focus on improving the patient's experience of care, including quality and satisfaction, and reducing the need for hospitalizations and urgent care visits.
  • Establish and communicate protocols and standards of care for a culturally and demographically diverse patient/family population.
  • Evaluate clinical outcomes data and analyze practice patterns to promote organizational responses that meet the needs of patients with specific complex and chronic illnesses.
  • Develop policies, standards, and protocols for the application of evidence-based practice and evaluate their impact on care management outcomes.
  • Identify and develop new programs designed to improve care outcomes for complex patients.
  • Educate, mentor, coach, and develop clinical staff and integrate complex illness management expertise into practice to improve quality of care, reduce transition problems from hospital to home, and reduce hospitalization rates.
Requirements:
  • License and current registration to practice as a Registered Professional Nurse in the State of New York within 6 months.
  • Clinical certification in a specific specialty, such as diabetes, oncology, wound/ostomy/continence, etc.
  • Bachelor's Degree in Nursing required; Master's Degree in Nursing preferred.
  • Proficient with personal computers, including Microsoft Office Word and Excel.
