Aveanna Healthcare LLC - Licensed Practical Nurse (LPN/LVN) Job Description

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team at Aveanna Healthcare LLC. As a LPN/LVN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver exceptional outcomes.

Responsibilities:
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
  • Participate in and update the nursing care plan to ensure patient needs are met.
  • Take appropriate nursing action based on assessment and achieve expected outcomes.
  • Recognize changes in patient needs and responses requiring intervention, implementing care to prevent risk or reduce risk.
  • Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
  • Provide care utilizing infection control measures that protect both staff and patient according to OSHA standards.
  • Educate patients and families regarding disease process, self-care techniques, and prevention strategies to meet patient needs.
  • Maintain knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills, and skill evaluations.
Requirements:
  • 6 months experience as a practicing nurse (new graduates welcome to apply for our excellent New Grad Program).
  • Nursing License (active and in good standing with the board).
  • CPR Certification (current, hands-on BLS).
