The Director of Case Management is responsible for leading the development and implementation of case management programs at Salem Hospital, Inc. This role requires strong leadership and management skills, as well as the ability to analyze complex data and make informed decisions.
Key Responsibilities:- Develop and implement case management programs to improve patient outcomes and reduce healthcare costs
- Lead a team of case managers and other healthcare professionals to ensure effective program implementation
- Analyze data to identify trends and areas for improvement, and make recommendations for program enhancements
- Collaborate with other departments, such as nursing and finance, to ensure seamless program integration
- Develop and maintain relationships with external partners, such as payers and vendors
- Master's degree in a related field, such as nursing or healthcare administration
- Minimum 7 years of experience in case management or a related field
- Strong leadership and management skills, with experience leading teams and managing budgets
- Excellent analytical and problem-solving skills, with the ability to interpret complex data and make informed decisions
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
- Minimum 5 years of experience in case management or a related field, with a focus on leadership and management
- Experience with data analysis and program evaluation
- Knowledge of healthcare regulations and policies
