Nurse Practitioner

Job Title: Primary Care Registered Nurse

Job Summary:

Mass General Brigham is seeking a Primary Care Registered Nurse to support our practice onsite. The successful candidate will provide professional nursing care to patients in a family medicine setting, working collaboratively with physicians, advanced practitioners, and other healthcare professionals to deliver patient-centered care.

Responsibilities:

* Triage clinical questions via phone calls
* Provide clinical advice and education to patients based on assessment findings and established protocols
* Provide appropriate care coordination for patients such as scheduling appointments, follow-up care and referrals to specialists and other healthcare services
* Follow-up on emergency room visits
* Collaborate with physicians, advanced practitioners, and other healthcare team members to ensure comprehensive patient care
* Collaborate with healthcare team members to develop and implement individualized care plans
* Provide direct patient care, including assessments, planning, implementation, and evaluation of care plans
* Administer Injections, medications, treatments, and procedures as prescribed by physicians and advanced practitioners and assist in minor surgical procedures

Requirements:

* Graduate of an approved school of nursing
* Current Massachusetts RN licensure
* Ambulatory/Primary care Experience Preferred
