Job SummaryWe are looking for a compassionate and experienced Registered Nurse to provide high-quality patient care in a long-term care setting. As a Long Term Care Specialist, you will be responsible for delivering individualized care to patients, collaborating with healthcare teams, and maintaining accurate records.
Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans, taking into account patient needs and preferences
- Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive patient care
- Maintain accurate and up-to-date patient records, including medical histories and treatment plans
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care
- Current RN licensure in the state of Pennsylvania
- Minimum 1 year of experience in long-term care or a related field
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Ability to work independently and as part of a team, with a focus on delivering high-quality patient care
