Clinical Care Coordinator

Job Summary:
Central Carolina Hospital is seeking a skilled Clinical Care Coordinator to join our team. As a Clinical Care Coordinator, you will play a vital role in ensuring the smooth transition of patients from our hospital to their homes or other healthcare facilities. This is a challenging yet rewarding position that requires strong communication and organizational skills, as well as a passion for delivering high-quality patient care.
Key Responsibilities:
• Coordinate clinical care to include discharge planning, medical necessity, and length of stay management
• Collaborate with healthcare providers, nurses, and other staff members to achieve optimal clinical outcomes
• Execute clear, complete, and concise documentation of all interventions and assessments
• Engage in practice that complies with regulatory standards, patient safety, and performance improvement initiatives
Requirements:
• Bachelor's degree in a related field (Master's degree preferred)
• Registered Nurse or Licensed Practice Nurse in North Carolina (or compact state)
