About the Role:

We are seeking a skilled Nurse Practitioner to join our team at EasyHealth, a leader in proactive and comprehensive healthcare. As a Nurse Practitioner, you will play a vital role in delivering preventive care and educational services to our members in the comfort of their own homes.

Key Responsibilities:
  • Perform comprehensive assessments, including Annual Wellness & Quality Gap Care visits, for our members in their homes.
  • Accurately code, verify, and document clinical findings.
  • Complete a detailed evaluation of the medical and social assessment of the beneficiary members using our proprietary tool or client EMR.
  • Identify and refer members/patients for routine, urgent, and emergent case management.
  • Make recommendations for member's follow-up with their primary care physician.
Requirements:
  • 2 years of hands-on experience as a Nurse Practitioner.
  • Current, unrestricted license in the state(s) in which the services are rendered.
  • Board-certified, preferably geriatric or adult medicine specialty.
  • Familiarity with the HCC model and quality HEDIS and other STAR measures.
  • Effectively communicate with elderly and chronically ill members.
  • Proficient in the use of computer, tablet, and mobile technology.
  • Willingness to travel within the geographic area.
What We Offer:
  • Starting from $120 per completed visit.
  • Payment for same day no show visits.
  • Independent Contractor.
  • Reimbursement for travel mileage.
