Responsibilities- Evaluate and monitor health status, performing diagnostic and therapeutic services as needed
- Provide age-appropriate education and counseling for disease prevention, disease management, and health maintenance
- Collaborate with Physicians to coordinate a continuum of care for patients
- Establish and maintain harmonious and cooperative working relationships with other healthcare professionals
- Effectively communicate with patients, family members, Physicians, and support staff
- Registered Nurse with additional training through an advanced studies program
- Experience in ambulatory care/primary care setting
- Licensed to practice within the State of Wisconsin and certified as a Certified Nurse Practitioner
- Family practice experience
- Knowledge of current practice standards for Nurse Practitioners
- Ability to provide age-appropriate care to patients of all ages
- Ability to effectively teach health promotion and disease prevention behaviors
- Ability to exercise good judgment and accept personal responsibility
This role requires the ability to meet the physical demands of primary care practice, including prolonged standing, walking, and reaching. The work environment is typical of a clinical practice setting, with moderate noise levels and occasional exposure to risk of blood-borne diseases.
