Clinical Social Worker (LCSW)

About the Role

We are seeking a highly skilled Clinical Social Worker to join our team at Care New England Health System. This is a unique opportunity to work with a talented interdisciplinary team to provide exceptional patient care.

Key Responsibilities:
  • Provide direct clinical social work services to patients and their families in the hospital setting, including psychosocial assessments, crisis intervention, counseling, and coordination of referrals.
  • Collaborate and coordinate care with the interdisciplinary team, attending daily rounds and providing feedback and recommendations to supplement patient care.
  • Maintain accurate documentation, including clinical assessments and follow-up notes in patient medical records.
  • Develop and maintain a network of community agencies to facilitate patient access and services upon discharge from the hospital.
Requirements:
  • A Master's degree in Social Work or a related field.
  • Certification as a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC).
  • Licensure as an Independent Clinical Social Worker (LICSW) is strongly preferred.
Estimated Salary Range:

$65,000 - $85,000 per year, depending on experience.
