Job Description

Job Summary: We are seeking a skilled and compassionate Registered Nurse to join our Care Team at Sanzie Healthcare Services Inc. as a Home Health Registered Nurse.

Key Responsibilities:

  • Monitor and supervise the care of patients with complex health needs, ensuring high-quality services are provided.
  • Review referrals from care coordinators, evaluate patient needs, and assign appropriate staff to provide necessary care.
  • Develop and revise patient care plans as needed, communicating changes to staff and ensuring seamless care delivery.
  • Supervise personal support aides and licensed practical nurses, providing guidance and support as needed.
  • Complete clinical records, including progress notes and supervisory visit entries, to ensure accurate documentation.
  • Review and co-sign clinical record entries made by personal support aides and licensed practical nurses, ensuring compliance with regulations.
  • Communicate patient care needs with other healthcare providers, patients, and caregivers, ensuring a collaborative approach to care.
  • Make recommendations to care coordinators regarding patient services, ensuring the best possible outcomes.
  • Conduct face-to-face visits with patients to re-evaluate their needs and provide personalized care.

Requirements:

  • Current and unencumbered license to practice as a Registered Nurse in Georgia.
  • Minimum of two years' experience in home health services or a related field, with a preference for experience working with older populations.
  • Ability to supervise personal support aides and communicate effectively with patients, families, and staff.
  • Working knowledge of the Rules and Regulations for Private Home Care Providers and the policies and procedures of the CCSP, SOURCE, ICWP, and the GAPP Medicaid Waiver Program.

Education/Training:

Specialized training is required for all personal care, and a license in the practice of nursing is essential for this role.
