Position Summary:
This role involves delivering nursing care under the guidance and oversight of a registered nurse and/or licensed physician, ensuring accountability for the quality of nursing services provided by oneself or those under supervision.
The position entails observing, contributing to the care plan, implementing, reporting, and documenting patient care in alignment with departmental policies, standards of care, and state regulations.
Utilizing specialized knowledge, judgment, and nursing skills, the candidate will assess data and plan, provide, and evaluate care tailored to the physical and developmental needs of assigned patients.

3 12-hour Night shifts.
Key Responsibilities:
1. Observes, compiles, and reports on the patient's physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs, utilizing information gathered from healthcare team members.
2. Contributes to the care plan under the direction of a registered nurse, including discharge planning, by utilizing assessment data and input from patients, families, and the healthcare team.
4. Implements care based on delegated interventions outlined in the care plan and medical orders, adhering to the scope of practice.
5. Provides care in accordance with physician orders and the nursing care plan, ensuring compliance with policies, procedures, standards of care, and regulatory requirements.
6. May delegate and supervise care provided by non-licensed personnel, following state regulations.
7. Promotes continuity of care by effectively communicating patient status to other caregivers.
8. Reports any changes in the patient's condition, including signs and symptoms, to the registered nurse and/or physician. Monitors and documents patient progress towards goals and expected outcomes in collaboration with the healthcare team.
10. May coordinate the admission process, including evaluations of new residents, and communicate effectively with residents, families, and referral sources regarding admission procedures.
12. Strives to provide all customers of Foundation Health with an excellent service experience by consistently demonstrating core and leadership behaviors.
Qualifications:
Must possess a valid current or temporary LPN license. An Associate's degree in nursing is required. BLS certification is mandatory, and additional certifications such as ACLS or PALS may be required based on the area of practice.
Creative thinking, prioritization, and flexible problem-solving skills are essential. The ability to communicate clearly in both verbal and written forms is also required.
PREFERRED QUALIFICATIONS:
Experience in the designated clinical area is preferred. Additional related education and/or experience is also preferred.
