About the RoleWe are looking for a Physical Therapist who is passionate about delivering high-quality care to patients in the comfort of their own homes. As a member of our team, you will be responsible for assessing patients' physical needs, developing and implementing individualized care plans, and providing manual therapeutic exercises, gait training, and other interventions as needed.
Responsibilities- Assess patients' physical needs and develop individualized care plans
- Provide manual therapeutic exercises, gait training, and other interventions as needed
- Continually assess and revise care plans as necessary
- Participate in multidisciplinary care planning as appropriate
To be successful in this role, you will need to have a current license to practice as a Physical Therapist in the state of PA and previous home healthcare experience is welcomed but not required. You will also need to be able to travel to cases as assigned.
