Lone Star Circle of Care is a non-profit, Federally Qualified Health Center (FQHC) dedicated to providing high-quality healthcare to patients in Central Texas. We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team.
Key Responsibilities- Perform duties under the direction of licensed medical personnel and/or Registered Nurses to assist in the examination and treatment of patients.
- Collect data and perform focused nursing assessments of the health care status of individuals in accordance with the Nurse Practice Act.
- Obtain, evaluate, and record patient history in medical files.
- Obtain and measure vital signs and record information on patients' charts.
- Assist in the examination of patients under the direction of licensed medical staff and/or Registered Nurses.
- May be required to draw and collect blood samples from patients and prepare specimens for laboratory analysis.
- Prepare and administer medications according to the five rights of medication administration.
- Perform routine diagnostic and laboratory tests.
- Referrals – send referrals, close out referral when consultations are received in clinic.
- Take incoming STAT calls from Quest and ARA, document, and send to provider.
- Complete data entry into Nextgen, state lab results, Hemoglobin, and leads.
- Research companies that offer services (Speech Therapy/Occupational Therapy/Physical Therapy) that the physician has referred, that is in network with the patient's insurance and request authorization.
- All other duties as assigned.
- Prepare and maintain examination/treatment rooms, including cleaning and sterilizing instruments, and inventory of medical supplies and materials.
- As needed, complete prior authorizations for medication.
- Assist when necessary with the processing of faxes and correspondence that come into the clinic.
- Synagis: place order and send authorization to insurance company, call parent to bring child in monthly for administration (mostly during flu season).
- Process medical records for urgent referrals.
- Completion of incident reports as needed.
- Explain treatment procedures to patients.
- Achieve and maintain clinical competencies as set forth by the Quality Council.
- Interpret and apply standing orders as defined by the Medical Executive Committee and Quality Council.

- Notifiable conditions reporting to state (STD, TB, Varicella, MMR – others as needed).
- Count vaccines weekly and monthly for State reporting.
- Graduate of an accredited vocational nursing school.
- Valid State of Texas Licensed Vocational Nurse (LVN) license.
- Basic Life Support (BLS) certification from American Heart Association or American Red Cross required.
- Previous nursing experience in a Clinic/Outpatient environment.
- Previous work using Electronic Health Records (EMR).
- Bilingual English/Spanish language skills.
