Cancer Care Navigator Role

About the Role
We are seeking an experienced Oncology Nurse Navigator to join our team. The ideal candidate will possess strong clinical expertise, excellent communication skills, and a passion for delivering patient-centered care. In this role, you will work closely with multidisciplinary teams to provide navigation services, develop personalized care plans, and facilitate effective communication among healthcare providers, patients, and families.
Responsibilities
The Cancer Care Navigator will be responsible for:
- Triage patient concerns and coordinate referrals to relevant specialists and support services.
- Develop and implement individualized care plans to meet the unique needs of patients and their families.
- Foster open communication among healthcare providers, patients, and caregivers to ensure informed decision-making and timely interventions.
- Maintain accurate records and track patient outcomes to identify areas for quality improvement.
Requirements
To succeed as a Cancer Care Navigator at Providence, you must have:
- A current Oregon Registered Nurse License.
- National Provider BLS certification from the American Heart Association.
- 3+ years of Oncology experience, preferably in a navigation or case management role.
- OCN certification within 18 months of hire (preferred).
