About the Role

Key Responsibilities
  • Coordinate patient care across various settings, including outpatient infusion clinics and hospitals
  • Develop and implement individualized care plans, taking into account patients' medical histories, social determinants, and treatment preferences
  • Collaborate with interdisciplinary teams to ensure timely and effective care transitions
