Job Summary

At Prisma Health, we're committed to transforming healthcare for the benefit of our communities. As a Patient Observer, you'll play a vital role in ensuring patient safety and well-being. Your primary responsibility will be to provide constant observation and support to patients who require close monitoring.

Key Accountabilities

* Provide one-on-one observation of assigned patients, documenting and reporting any risks, changes, or problems observed to prevent self-harm and harm to others (30%)
* Conduct safety sweeps as per policy when observing patients at risk for self-harm (15%)
* Ensure patients are not left unattended and notify unit staff and/or Security if a patient tries to leave the floor (15%)
* Participate in hand-off communication (report) regarding patients assigned to observe (15%)
* Assist with meeting patient needs, including turning and positioning, patient-centered diversional activities, feeding, ambulation, patient transfers, and transports (15%)
* Maintain constant observation of patients at risk for self-harm, including during bathing, toileting, and sleeping (10%)

Requirements

* High school diploma or equivalent preferred
* No previous experience required; previous experience in a healthcare setting preferred
* CNA or PCT certification preferred
* Evening shift
