About the Role: As an Advanced Practice Clinician, you will have the opportunity to work with a talented team of healthcare professionals who share your passion for delivering exceptional patient care. You will provide direct, hospital-level internal medicine care to patients in their homes and create and implement multidisciplinary plans of care for patients on Advanced Care, Extended Care, and Bridge Care service lines.

Job Summary: DispatchHealth Management is seeking an experienced Advanced Practice Clinician to join our team as a Hospitalist in-home provider. In this role, you will work closely with our team to deliver high-quality, hospital-level care to patients in their homes.

Key Responsibilities:

  • Provide direct, hospital-level internal medicine care to patients in their homes
  • Create and implement multidisciplinary plans of care for patients on Advanced Care, Extended Care, and Bridge Care service lines
  • Collaborate with DispatchHealth Management APPs, including education and engagement around Advanced Care program services
  • Participate in program enhancement for Advanced Care, Extended Care, and Bridge Care service lines

Requirements:

  • Graduate with an advanced degree from an accredited institution
  • 2-3 years of experience in hospitalist or equivalent experience
  • Current unrestricted state license as a Nurse Practitioner or Physician Assistant
  • Nationally board certified, current
  • Current BLS / ACLS certification
  • Prescriptive authority and DEA
  • Ability to lift and carry equipment up to 50 pounds
  • Ability to walk up and down several flights of stairs easily while carrying equipment
  • Valid driver's license with clean driving record
  • Ability to work a varied schedule with evenings, holidays, and weekends required

Salary: The estimated salary for this role is $145,000 - $165,000 per year, depending on experience.
