About the Role: As an Advanced Practice Clinician, you will be responsible for delivering high-quality, hospital-level care to patients in their homes. You will work with a talented team of healthcare professionals who share your passion for delivering exceptional patient care.

Key Responsibilities:

  • Deliver high-quality, hospital-level internal medicine care to patients in their homes
  • Create and implement multidisciplinary plans of care for patients on Advanced Care, Extended Care, and Bridge Care service lines
  • Collaborate with DispatchHealth Management APPs, including education and engagement around Advanced Care program services
  • Participate in program enhancement for Advanced Care, Extended Care, and Bridge Care service lines

Requirements:

  • Graduate with an advanced degree from an accredited institution
  • 2-3 years of experience in hospitalist or equivalent experience
  • Current unrestricted state license as a Nurse Practitioner or Physician Assistant
  • Nationally board certified, current
  • Current BLS / ACLS certification
  • Prescriptive authority and DEA
  • Ability to lift and carry equipment up to 50 pounds
  • Ability to walk up and down several flights of stairs easily while carrying equipment
  • Valid driver's license with clean driving record
  • Ability to work a varied schedule with evenings, holidays, and weekends required
