Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans to improve health outcomes.
Key Responsibilities- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment in patients' homes
- Collaborate with physicians, case managers, and other healthcare professionals to develop and implement care plans
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Document patient information and care plans in accordance with Matrix Medical Network's guidelines and protocols
- Master's degree in Nursing or related field
- Current RN and NP licensure in the state of practice, including prescription authority
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent communication and interpersonal skills
