About the Role:

Nurse Practitioners play a vital role in Matrix Medical Network's mission to improve the quality of care for at-risk individuals. As a Home Risk Assessment Nurse Practitioner, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and providing personalized care plans to improve health outcomes.

Key Responsibilities:

Conduct adult/geriatric assessments, medical history, diagnosis, and treatment
Provide health education and collaborate with primary care physicians on patient care
Work collaboratively with case managers, social workers, and other healthcare professionals to ensure comprehensive care
Conduct assessments in various settings, including home visits, skilled facilities, and telehealth visits

Requirements:

Master's degree in nursing or equivalent experience and licensure
Current RN and NP licensure with prescription authority or ability to obtain
Board certification in a Matrix-approved specialty
1 year of experience as a Nurse Practitioner
Strong computer skills and familiarity with electronic health records
Excellent verbal and written communication skills
