We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Aveanna Healthcare. As a LPN, you will provide high-quality patient care in a home environment, working closely with our patients and their families to ensure the best possible outcomes.
Responsibilities:- Provide one-to-one patient care in a home setting, executing physician-prescribed plans of care and documenting all activities and assessments in a timely manner.
- Administer prescribed medication, treatments, and therapies, as well as coordinate care and provide health education to patients and their families.
- Participate in the development and implementation of individualized care plans, working collaboratively with our interdisciplinary team to ensure seamless care transitions.
- Conduct thorough patient assessments, identifying and addressing any changes in condition or care needs, and reporting these changes to the supervisor and/or physician as necessary.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- One to six months of recent experience as a Licensed Nurse in a clinical care setting, with a preference for home health experience.
- Competitive hourly pay and weekly paychecks.
