Position Overview:
The MDS Coordinator plays a vital role in the Skilled Nursing Facility (SNF) and Long Term Care (LTC) setting, focusing on the comprehensive assessment of residents. This position requires a proactive approach to ensure the accurate completion of the Minimum Data Set (MDS) and the development of effective care plans.
Key Responsibilities:
- Lead and facilitate the assessment process for residents, ensuring timely and thorough completion of the MDS.
- Regularly review and update facility policies and procedures related to MDS and care planning.
- Support residents in formulating the care plan components of their discharge strategies.
- Collaborate with the Director of Nursing Services to establish communication protocols for new resident admissions.
- Assist in organizing and scheduling meetings for resident assessments and care planning.
- Evaluate treatment options and recommend activities tailored to the medical and social backgrounds of residents.
- Engage in the continuous development and review of resident assessments and care plans, including quarterly and annual evaluations.
Qualifications:
- Current RN license in Ohio is required.
- Experience in long-term care settings is essential.
- Proven track record as an MDS Coordinator is necessary.
- Familiarity with MDS 3.0 is a must.
