Patient Care Coordinator

We are seeking a dedicated Case Manager for our Healing Center, who will play a crucial role in providing comprehensive support to patients and their families. This position involves a variety of responsibilities aimed at ensuring the safety and well-being of all individuals involved in the treatment process.
Key Responsibilities:- Facilitate Supportive Groups: Lead or assist in educational and supportive sessions for patients and their families, ensuring a safe environment for all participants.
- Collaborate with Treatment Teams: Maintain ongoing communication with the treatment team, sharing essential patient information and insights to enhance care continuity.
- Evaluate Interventions: Assess the effectiveness of social service interventions and adjust strategies based on patient and family feedback during team discussions.
- Professional Commitment: Uphold a high standard of professional performance by balancing patient care with organizational responsibilities.
- Conduct Assessments: Complete thorough assessments within specified timeframes, identifying patient needs and readiness for therapy.
- Plan Education Initiatives: Develop and implement educational strategies for families based on assessment data, ensuring effective communication of discharge plans.
- Discharge Planning: Collaborate with interdisciplinary teams to document and communicate discharge needs, ensuring a smooth transition to aftercare.
- Provide Therapeutic Activities: Organize and facilitate therapeutic activities that promote group cohesion and individual participation.
- Maintain Professional Growth: Engage in continuous professional development through seminars and learning activities to stay updated on industry trends.
- Bachelor's Degree in Psychology or a related field is required.
- A minimum of two years of experience in a clinical setting is preferred, including field practicums.
- Experience in treating Psychiatric Illness, Substance Abuse Disorders, or Dual Diagnosis Disorders is highly desirable.
