Kings View Corporation, a nonprofit organization dedicated to delivering behavioral health services to marginalized communities, is seeking a dedicated Care Manager to further our mission of providing empathetic support to individuals with limited access to resources. The ideal candidate will possess a strong commitment to social equity, a deep respect for human dignity, and a proactive approach to addressing community needs.
Role Responsibilities:
The Care Manager will engage directly with Members and their families, Authorized Representatives (ARs), caregivers, and other authorized support individuals. This role involves collaboration with a multidisciplinary care team to identify and address gaps in care, ensuring comprehensive coordination of primary, behavioral, developmental, oral health, long-term services and supports (LTSS), and other services that impact social determinants of health (SDOH).
Key Contributions:
- Conduct outreach and education to vulnerable populations, focusing on health promotion and recovery.
- Develop and implement care management plans tailored to individual needs.
- Coordinate with healthcare and social service professionals to facilitate access to essential resources, including medical care and financial assistance.
- Advocate for Members within the healthcare system to ensure their needs are met.
- Utilize motivational interviewing and trauma-informed care approaches in interactions.
- Assist with discharge planning in collaboration with hospital staff.
- Monitor treatment adherence and provide health promotion training.
- Engage in crisis intervention when necessary, ensuring safety and well-being.
Qualifications:
- High School Diploma or equivalent required; BA or BS preferred.
- Minimum of one year of experience with target populations such as the homeless, those with severe mental illness, or individuals involved in the criminal justice system.
- Valid Class C Driving License.
- Knowledge of behavioral health systems, criminal justice processes, and housing challenges.
- Ability to effectively communicate and build relationships with diverse stakeholders.
- Flexibility to work evenings and weekends as needed.
- Bilingual English/Spanish skills are a plus; experience as a Certified Nursing Assistant (CNA) or Medical Assistant (MA) is desirable.
- Must obtain a National Provider Identification (NPI) number prior to employment.
