Job Summary
We are seeking a skilled Social Worker Director to join our team. The successful candidate will be responsible for supporting the healthy emotional and social adjustment of residents, identifying and understanding the social and emotional factors underlying resident's mental illness, and communicating these factors to the treatment team.
Key Responsibilities
- Support residents' emotional and social adjustment
- Identify and communicate social and emotional factors underlying mental illness
- Assist residents, families, and employees in understanding psychiatric illness and accepting necessary treatment
- Work with the MIDS coordinator to complete designated sections leading to completion of the MDS
- Participate in care plan meetings and document quarterly reviews and interim notes
- Ensure transfer and discharge rights are respected
- Facilitate room changes and conduct resident council meetings
- Function as Grievance Officer and comply with mandated reporter obligations
- Coordinate the admission process and ensure PASRR is completed before admission
- Ensure guardianship paperwork is up to date and provide medical certificates as requested
- Provide Level of Care screenings and participate in discharge planning
- Attend management meetings and supervise the PT Community Service Coordinator
Requirements
- Current, active license to practice as a Social Worker in Massachusetts
- MSW degree preferred
- Effective interpersonal skills and ability to work with an interdisciplinary care team
- Basic understanding of computer technology and proven written and oral communication skills
- Proven decision-making and analytical skills
- Able to relate positively with others, including employees, residents, family members, and personnel of outside agencies
- Meet all local health regulations and pass post-employment physical exam
- Meet requirements to provide social services in long-term care facilities
Company Compliance
- Must adhere to the Company's Code of Conduct and business Ethics policy
Staff Development
- Attend and participate in educational activities, in-service training, and staff meetings
Quality Improvement
- Participate in Quality Improvement activities as assigned
Language
The successful candidate will be proficient in American Sign Language (ASL) and will collaborate with the communication department to ensure proper communication with residents, family members, caregivers, and community agencies.
