We are seeking a qualified Licensed Practical Nurse (LPN) to provide direct patient care to our residents and patients in a skilled nursing and rehabilitation setting.
Key Responsibilities:- Provide regular resident status updates to appropriate personnel.
- Conduct and document thorough evaluations of each resident's medical status upon admission and throughout their course of treatment.
- Assist in implementing individualized treatment plans for each assigned resident.
- Complete required documentation in an accurate and timely manner.
- Oversee Falls Program and Pressure Sore Program for residents on their team.
- Perform routine nursing services for residents as directed.
- Must be able to read, write, speak, and understand the English language and possess good communication skills.
- Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
- Must be able to relate information concerning a resident's condition.
- Must possess a current, unencumbered, active state license to practice as an LPN.
- Experience in Skilled Nursing/Rehabilitation facilities preferred.
