We are currently seeking a Licensed Practical Nurse to join our team at our Skilled Nursing Facility in Portsmouth, VA. As an LPN, you will work closely with our experienced nursing staff to provide exceptional patient care.

About the Salary and Benefits

This is a part-time position paying $40 per hour, with flexible scheduling and competitive pay. In addition to your hourly rate, you will also receive a range of benefits, including telemedicine, mental health, well-being support, essential health and wellness benefits, paid time off, weekly pay, and a 401k plan available.

We require a minimum of one year of experience in a long-term care facility. If you have excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care, we encourage you to apply.

You will be responsible for performing designated job duties under the direction of a Registered Nurse while maintaining the LPN scope of practice. This includes implementing individualized plans of care, administering medications, monitoring patient outcomes, and documenting patient status according to federal and state regulations.

To be successful in this role, you should possess a completion of an accredited practical nursing program and maintain educational requirements as required by the Virginia Board of Nursing.

Responsibilities:
  1. Performs designated job duties under the supervision of a Registered Nurse.
  2. Participates in the development of an individualized interdisciplinary plan of care.
  3. Implements the individualized interdisciplinary plan of care according to biophysical, psycho-social, and age-specific needs.
  4. Monitors patient/resident outcomes and reports abnormal findings to the Assistant Clinical Manager/Supervisor.
  5. Documents patient's status according to the unit-specific procedure.
Education Requirements:
  • Completion of an accredited practical nursing program.
