Position Title: Transition Navigator

Overview: The Transition Navigator plays a crucial role in supporting youth and young adults, particularly those with experiences in the foster care system. This position is dedicated to fostering positive change and enhancing the lives of individuals aged 14 to 29.

Mission: At The New Foster Care (TNFC), our commitment is to transform the Michigan Foster Care system and create a significant impact on the lives of youth. We achieve this through collaboration with state and local agencies, innovative program development, and advocacy for policy changes that prioritize the well-being of children.

Core Responsibilities:

  • Build trusting relationships with youth and their families to assess strengths and needs.
  • Provide hands-on coaching and support in essential life transition areas, including Employment, Living Situations, Education, Community Engagement, and Personal Well-being.
  • Assist in overcoming mental health challenges by offering tailored rehabilitative services and case management.
  • Facilitate access to community resources and educational opportunities, ensuring youth are informed about eligibility and application processes.
  • Implement trauma-informed care practices to address the unique experiences of youth.
  • Engage in ongoing professional development and participate in treatment team planning.

Qualifications:

  • Master's degree in social work, psychology, or a related field; a bachelor's degree may be considered with relevant experience.
  • Experience working with youth, particularly those in foster care, is preferred.
  • Strong communication skills, both verbal and written, to effectively interact with diverse stakeholders.
  • Ability to manage a flexible schedule to accommodate client needs outside of typical business hours.
  • Proficient in documentation and case management practices.

Additional Requirements:

  • Valid driver's license and reliable transportation for client visits.
  • Commitment to maintaining professional standards and ethical guidelines.
  • Willingness to undertake other responsibilities as assigned.
