Job Summary

We are seeking a compassionate and experienced Registered Nurse to join our Hospice team as a Case Manager. As a Hospice RN, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities
  • Conduct thorough assessments of patients' physical, functional, psychosocial, and environmental needs.
  • Develop and implement individualized plans of care, ensuring compliance with AdvisaCare Hospice policies and procedures, as well as Medicare regulations.
  • Provide education and support to patients and their families on disease management, self-care techniques, and end-of-life care.
  • Supervise paraprofessionals providing services to patients and ensure timely and accurate documentation of patient interactions.
  • Participate in interdisciplinary group meetings and contribute to the development, review, and revision of patient care plans.
Requirements
  • Current Michigan license in good standing.
  • Minimum 1 year of experience in home care or hospice.
  • Empathetic approach to patients and caregivers.
  • Ability to adapt to patients' status and needs on a full-time, on-call basis.
