Position Overview:

As a vital member of the Advanced Heart Failure and Transplant team at Intermountain Health, you will play a crucial role in delivering exceptional patient care. This position is designed for a Nurse Practitioner or Physician Assistant who is dedicated to improving patient outcomes and enhancing the overall healthcare experience.

Role Responsibilities:

  • Provide comprehensive patient assessments and develop treatment plans in collaboration with a multidisciplinary team.
  • Monitor and manage patients in both inpatient and outpatient settings, ensuring continuity of care.
  • Conduct follow-up visits and provide education to patients and their families regarding health management and disease prevention.
  • Perform procedures as per specialty guidelines and refer patients to appropriate healthcare providers when necessary.
  • Maintain accurate and timely documentation of all clinical interactions and patient care activities.

Qualifications:

  • Current licensure as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA).
  • Certification as a Nurse Practitioner or Physician Assistant is required.
  • Eligibility for state and federal controlled substance licenses.
  • BLS certification and any additional required certifications (e.g., ACLS, PALS).

Preferred Experience:

  • Experience in cardiology, cardiovascular surgery, or related fields is highly desirable.
  • New graduates with relevant clinical rotations are encouraged to apply.
  • A Master's degree or higher in a relevant field.
