As a Home Health Speech Language Pathologist, you will work closely with our multidisciplinary team to assess, diagnose, and treat patients with speech, language, communication, or swallowing difficulties.

Responsibilities:
  • Conduct thorough evaluations to assess patients' speech and language abilities.
  • Develop and implement individualized treatment plans to address patients' needs.
  • Provide one-on-one therapy sessions to patients in their homes.
  • Collaborate with other healthcare professionals to ensure comprehensive care.
  • Document patient progress and maintain accurate records.
Requirements:
  • Current license as a Speech Language Pathologist in New Hampshire.
  • Certificate of clinical competence in speech from the American Speech and Hearing Association.
  • Solid computer skills and experience with electronic medical records (EMR).
