Job Description

This Hospice RN Case Manager position plays a crucial role in helping patients and families navigate the hospice care experience with the support they deserve. Key responsibilities include assessing and addressing physical, psychosocial, and environmental needs, conducting thorough initial and ongoing evaluations, providing regular health updates, developing personalized care plans, and maintaining quality of care by following individualized plans focused on achieving positive health outcomes.

Required Skills and Qualifications

To be successful in this role, you will need to possess a Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Bachelor's), current State RN license, and minimum of 1 year of clinical experience, preferably in hospice or home health. You must also have strong nursing skills and familiarity with accepted nursing standards, as well as the ability to manage multiple priorities, deadlines, and documentation requirements.
