Geriatric Care Professional

About the Job:

In this role, you will work as a vital member of the PACE Interdisciplinary Team at Element Care. Under the guidance of a licensed nurse, you will participate in all aspects of personal care and safety for our participants at both our ADH site and in the community.

Key Responsibilities:
  1. Assist participants with personal care tasks at the site and in the community, as directed by nursing staff.
  2. Support meal services, including one-on-one feedings, while ensuring adherence to dietary restrictions and promoting healthy eating habits.
  3. Collaborate with nurses to collect specimens, monitor oxygen levels, and record vital signs.
  4. Communicate any medical concerns or changes in participants' physical, cognitive, or functional status to primary nurses promptly.
  5. Accompany participants to medical appointments and other important events as needed.
  6. Perform daily assignments and restock supplies to maintain a clean and safe environment.
Requirements:
  • A high school diploma or equivalent is preferred.
  • HHA certification or an equivalent qualification is required.
  • At least one year of experience working with seniors is necessary.
  • You must be able to establish strong relationships with elderly individuals and anticipate their needs effectively.
  • Your duties will include taking vital signs, transferring participants safely, and communicating participant updates.
  • Excellent verbal and written communication skills are essential.
  • A cooperative attitude and ability to work positively within a team are mandatory.
  • Intermediate Microsoft computer skills are beneficial.
  • Frequent local travel is required.
Compensation and Benefits:

This position offers a competitive salary of $45,000 - $55,000 annually, depending on experience, along with benefits that support your well-being and career growth.
