We are seeking a compassionate and skilled Social Services Specialist to join our team at Spring Brook Center, Inc. As a key member of our interdisciplinary team, you will work closely with patients, residents, and their families to identify strengths, social, emotional, and mental health needs.

Your goal will be to provide patients with the highest practical level of physical, mental, and psychosocial well-being and quality of life. You will collaborate with our team to promote and protect patient rights and well-being, educate staff on the role of social services, and develop written plans of care to address social service needs.

Responsibilities include:

  • Conducting patient/resident assessments and developing individualized plans of care
  • Collaborating with the interdisciplinary team to promote patient/resident well-being and quality of life
  • Providing education and support to patients, residents, and their families
  • Coordinating discharge planning activities, including referrals to community-based agencies and services
  • Maintaining relationships with community-based agencies and services to benefit our patient/resident population
