Hospice Plus is seeking a highly skilled and compassionate Patient Care Manager to join our team. As a key member of our leadership team, you will be responsible for the day-to-day operations of our hospice sites, ensuring the provision of high-quality care to our patients.
Key Responsibilities- Manage and oversee clinical and administrative functions, ensuring seamless operations and exceptional patient care.
- Respond to phone inquiries and referrals in a timely, professional, and compassionate manner.
- Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
- Assume site management responsibilities in the absence of the Executive Director, Administrator, or Senior Patient Care Manager.
- Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing.
- Bachelor's Degree in Nursing preferred from an accredited school of nursing.
- Three years' experience in direct patient care in a clinical setting required.
- Minimum one year of management/supervisory experience required.
- Two or more years' experience in hospice or home care required.
- Current license to practice nursing in the state of requested employment.
- Sensitivity to the feelings of the terminally ill and their families.
- Ability to demonstrate flexibility and perform a variety of tasks with efficiency and composure.
- Effective communication skills with people of all socioeconomic backgrounds.
- Ability to influence people's opinions, attitudes, or judgments.
- Display evidence of patient advocacy and responsible functioning in the workplace.
- Knowledge of terminally ill patients and their families, as well as understanding of hospice concept.
- Excellent patient assessment skills and good oral and written communication.
- Thorough knowledge of managed care principles, regulatory guidelines, and management principles.
