Job Summary

This role assists residents with mental health disabilities to transition into the community via participation in various community activities, enhancing their life experiences and providing support through development and implementation of community integration projects.

Key Responsibilities
  • Assist supervisors with development, organization, and facilitation of community inclusion activities, including volunteer opportunities and leisure activities.
  • Transport residents to and from community and recreational activities.
  • Coordinate activity schedules with other community inclusion team members to ensure residents have opportunities for volunteer and other activities.
  • Train and supervise residents in the completion of work/volunteer activities by modeling specific behaviors.
  • Promote positive community growth through collaboration with volunteer and vocational partners, networking in the community, and utilizing community resources to assist residents' growth and independence.
  • Communicate planned community activities to all Elwyn program staff through the distribution of the activity calendar and in staff meetings.
  • Document and maintain all necessary data pertaining to community inclusion activities.
  • Assist in the planning and coordination of Elwyn annual resident events.
  • Assist in the development of wellness and recovery plans, based on regulatory requirements.
  • Perform cardiopulmonary resuscitation (CPR), safe crisis intervention (SCM), and medication administration using agency-trained protocols.
Requirements
  • High School diploma or equivalent required; associate degree or 60 credits in a human service field preferred.
  • Experience working with people with behavioral health disabilities, preferred.
  • Community services experience, preferred.
  • Demonstrated ability to adapt to changes in workload, assignments, and schedule.
  • Ability to flex schedule to meet resident needs.
  • Demonstrated ability to work effectively as part of a team.
  • Demonstrated strong time management and organizational skills.
  • Must be able to perform CPR, SCM, and medical administration using agency-trained protocols.
  • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills.
  • Demonstrated basic experience with Microsoft Office applications, including Word, Excel, and Outlook.
  • Must have a current, valid driver's license in the state of residence, three years of driving experience in the United States, and an acceptable driving record.
