Key Responsibilities- Client Assessment and Care Planning: Conduct comprehensive initial and ongoing assessments of clients' needs and create individualized care plans to ensure the highest level of care and support.
- Care Coordination and Management: Coordinate and monitor total client care, ensuring adherence to best practices and promoting preventative care.
- Caregiver Supervision and Training: Supervise and train caregivers on care techniques, ensuring they have the necessary skills and knowledge to provide exceptional care.
- Communication and Collaboration: Communicate effectively with family members, caregivers, and other healthcare professionals to ensure seamless care coordination and support.
- Quality Assurance and Improvement: Audit client charts for compliance and participate in quality improvement activities to ensure the highest standards of care.
- RN or LPN license in MD or a compact license.
- 5+ years of experience in home health or elder care.
- Knowledge of MD COMAR regulations.
- Strong communication and organizational skills.
- Experience in training and managing caregivers, CNAs, med techs, and other staff.
- Ability to adapt positively and professionally to change.
- Valid Driver's License and Auto Insurance with a clean driving record.
