As an Inpatient Social Services Specialist, you will play a crucial role in the healthcare team, ensuring comprehensive care for patients during their hospital stay. Your expertise will be essential in navigating the complexities of patient needs and facilitating effective discharge planning.
Department: Inpatient Services
Schedule: PRN, including weekends and occasional holidays. No on-call duties required.
Key Responsibilities:
- Develop and coordinate a holistic care plan for patients, considering their individual preferences and values.
- Conduct ongoing assessments and evaluations to monitor patient progress and adjust care plans as necessary.
- Advocate for patients by identifying and addressing barriers to care and ensuring access to necessary resources.
- Collaborate with healthcare providers to implement and maintain effective health plans.
- Serve as a resource for accurate medical documentation and appropriate service delivery related to diagnoses and post-discharge care options.
Qualifications:
- A valid Maryland Social Worker license is mandatory.
- Candidates must possess a minimum of a LBW license or higher.
- A minimum of three years of relevant experience is preferred.
- An LMSW with at least two years of acute care experience in emergency departments, hospitals, or community settings is advantageous.
- Experience working with patients with complex medical conditions is a plus.
