Join Our Team as a Clinical Supervisor - Home Health RN

We are seeking a highly skilled and compassionate Clinical Supervisor - Home Health RN to join our team at Enhabit Home Health & Hospice. As a Clinical Supervisor, you will play a vital role in ensuring the delivery of high-quality patient care in the comfort of their own homes.

About the Role

The Clinical Supervisor - Home Health RN will be responsible for overseeing the care of patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans. You will also be responsible for mentoring and guiding our team of licensed professionals to ensure they have the necessary skills and knowledge to provide exceptional patient care.

Responsibilities
  • Facilitate effective communication between physicians, referral sources, patients, caregivers, and employees
  • Review and approve patient information submitted by licensed professionals during Start of Care, Recertification, Resumption of Care, or Evaluation visits
  • Provide oversight of patient care, assist with case conferences, and review and approve orders
  • Assist the Branch Director in providing ongoing education and training to branch clinicians to ensure understanding of documentation requirements to meet regulatory standards
Requirements
  • Graduate of an accredited school of professional nursing and licensed in the state of employment and where practicing
  • A minimum of two years nursing experience and one year experience in home health
  • Valid state driver's license and automobile liability insurance
  • Ability to drive an automobile in all types of weather conditions
